• Q: Are all fields mandatory?

  A: Most fields are mandatory. Those that are not, are indicated as optional. If you try to save a record without populating all of the mandatory fields, the system will display a notification telling you what is needed.

• Q: Will my data be saved if I accidentally close or refresh the page?

  A: If you have already saved your information, it will be saved and available when you log back in. Any data not yet saved, will be lost.

• Q: Can I change my submission?

  A: Yes, you can update information by logging back into the form and clicking on “Begin editing”.

• Q: I don’t have anything to disclose. Do I have to complete the form?

  A: Yes, it is important this information also be captured. Once you have logged in, please confirm your profile information, select “I have no items to disclose” and "Mark Complete".
