Electronic Intellectual Property and Commercialization Activity Disclosure Form

Preamble

The University of Waterloo Is known globally for its unique creator-own IP policy that serves to attract world class research talent and underpins a widely recognized entrepreneurial campus culture.

However, Waterloo has not historically been systematically cataloguing its intellectual property (IP) and related commercialization activity and this is compromising the Institution's ability to respond to increasing numbers of requests for such data from government and "ranking” agencies.

The Electronic IP and Commercialization Activity Disclosure form has been designed to facilitate the disclosure of IP and commercialization information on an annual basis in accordance with Policy 73. In order to expedite a collection of baseline data, this version of the form does not capture non-software copyright IP such as books, musical compositions and artworks. This will be achieved in a future version of the form.

The form should be updated annually by faculty members (including emeritus faculty with active research) and take no more than 10 minutes to complete.

Data provided remain confidential and will be used internally for management reporting purposes and in aggregate for other external reporting purposes. If you have any questions, please refer to the frequently asked questions or contact our support team.
